All that is assuming Innworld developed exactly the same as Earth, which would likely be at least partially incorrect. Magic would allow for the development of some things prior to what Earth industry allowed. Plus, while Innworld is nominally compared to medieval eras, the world there is tens of thousands of years old. That is a huge amount of time for things to be different in many aspects compared to Earth.

Regardless, while stuff like modern bras and panties might not be the same, they did have things such as breast wraps and loincloths and the like, all of which could be colloquially termed as ‘underwear’, especially when using modern terms in a written work.
